  • Receiving unnecessary surgery/diagnostic testing
  • Receiving an overdose of medication
  • Experiencing a delay in service
  • Receiving inadequate care or treatment by a Medicare hospital or doctor
  • Experiencing a change in condition that was overlooked and not treated
  • Receiving a misdiagnosis
  • Receiving inadequate discharge instructions

For Medicare Part A or Part B quality of care complaints (including hospital discharge notices), contact California's Quality Improvement Organization (QIO), Health Services Advisory Group (HSAG) online or by phone at 1-800-841-1602 or 1-800-881-5980 (TDD for the hearing impaired).
